Use channel flags CF_PROXY_MODE or CF_PROXY_MEDIA to determine if to disable
SOA when passing a T38 ReINVITE across legs when in proxy media or bypass media.
These channel flags are set when the sofia profile params inbound-bypass-media or
inbound-proxy-media are enabled as well as when the channel variables bypass_media or
proxy_media are set which should handle a few edge cases better.

While setting the IV for AES ICM the nonce is simply typecast from
a void * to a v128_t *. This breaches alignment requirements for
v128_t objects on platforms that require it.
Instead make a copy of the nonce to assure proper alignment.
In certain deployments it might be desirable to remove the User-Agent header from the SIP message.
Set the sofia profile parameter "user-agent-string" to "none" to suppress this header.
ADTRAN Total Access devices do not support sending the rport parameter in
the Via header. This allows us to detect the device and force rport when
using the "safe" parameter, enabling the device to be used behind NAT.
