xquanluuandClaude Opus 5 320cf28d08 fix: a caller-abandoned inbound call shows as live for 12 hours
When the CANCEL arrives before we finish fetching the application,
InboundCallSession's constructor calls _onCancel() - which reports the terminal
no-answer/487 - and then falls through to the unconditional Trying notification
a few lines below. Nothing guards it: notifiedComplete only covers Completed.

The webhooks are merely out of order, but the redis call record is written with
hmset, a MERGE, so the later write wins. Measured against the deployed
realtimedb-helpers by calling updateCallStatus in the same order the constructor
does:

    after 487:  callStatus=no-answer sipStatus=487 ttl=3600s  inActiveSet=false
    after 100:  callStatus=trying    sipStatus=100 ttl=43200s inActiveSet=true

So the record reverts to trying/100, its TTL jumps from the 1-hour
post-completion value to the 12-hour in-flight one, and sipStatus 100 re-adds the
call to the active-call sorted set. A call the caller gave up on before it was
ever answered is then reported as live by GET /Calls for the next 12 hours, and
counted among active calls.

Guarding the Trying notification on the same flag the branch above tests keeps
the change to one condition. The consequence is that such a call's first and only
status event is the terminal one, which is what actually happened.

Found while reviewing the sip_reason_header work, which touches the adjacent
line; kept separate because it changes the status-event sequence on this path and
deserves its own review rather than riding along.

Not covered by a test: the race needs the CANCEL to beat the application fetch,
which is not deterministically reachable from outside the cluster without a
non-blocking INVITE primitive in the harness. The redis consequence above is
measured; the SIP trigger is not.

jambonz-feature-server CI

This application implements the core feature server of the jambones platform.
